百令膠囊治療尿毒癥肺病的療效觀察

[Abstract]Objective To observe the therapeutic effect of bailing capsule on uremia and pulmonary disease. Methods This research study phase between January 2017 and January 2019, at this time select among our receiving the existence of uremic lung disease patients as the research object, a total of 60 patients, according to its treatment were divided into control group and research group, control group treatment for regular hemodialysis treatment, the team on the basis of regular hemodialysis treatment using the capsule treatment, analysis of the results of this study, two groups of patients with therapeutic effect, urea nitrogen, serum creatinine, and patients with various lung function index (FEV1, FEV1/FVC) as a reference index. Results (1)The treatment efficiency of the control group was 80%, the treatment efficiency of the study group was 96.67%, and the treatment efficiency of the study group was higher than that of the control group. (2)The levels of urea nitrogen and creatinine in the control group were (9.18±1.20) mmol/L and (242.46±27.20) mol/L, and the levels of urea nitrogen and creatinine in the study group were (8.40±0.87) mmol/L and (194.23±30.17) mol/L. (3)FEV1 and FEV1/FVC in the control group were (1.62±0.31) L and (49.12±8.68) % respectively, and FEV1 and FEV1/FVC in the research group were (2.48±0.29) L and (63.25±8.70) % respectively. The above items were analyzed by statistical software, P<0.05. Conclusion Uremia and pulmonary disease is a very serious disease. The treatment of uremia and pulmonary disease can be treated with bailing capsule on the basis of conventional treatment, which has a significant therapeutic effect and can promote the improvement of renal function status and lung function index level of patients. Therefore, it is worthy of clinical promotion and application.
尿毒癥發生原因為慢性進行性腎功能損傷所引起的，腎臟無法維持其基本功能，出現代謝方面的紊亂，機體內分泌失調，還會影響到其他臟器功能。機體肺部是最容易受到尿毒癥影響的臟器。肺臟受到的損傷大都是尿毒癥直接造成，也可以通過另一臟器影響，對肺部造成嚴重創傷，而且肺部損傷大都在尿毒癥晚期階段發生[1]，說明患者病情已經十分嚴重，需要采取及時、有效的治療措施。但是引起尿毒癥肺病的機制比較復雜，單一采取相應的尿毒癥和肺病治療效果不夠理想。百令膠囊在肺病、腎病的應用中均取得一定效果，本研究在以往基礎上分析百令膠囊在尿毒癥肺病中的應用效果，具體內容如下。
1 資料與方法
1.1 一般資料：本研究研究階段選擇2017年1月至2019年1月，在這個時間段當中選擇我院接收的存在尿毒癥肺病的患者作為研究對象，共有60例，根據其治療方法分為對照組和研究組。對照組：男性病例有20例、女性病例有10例，年齡分布區間為38~65歲，年齡均值為（42.1±3.7）歲，患尿毒癥時長為1~4年，平均時長為（1.3±0.6）年；研究組：男性病例有19例、女性病例有11例，年齡分布區間為36~65歲，年齡均值為（43.5±3.5）歲，患尿毒癥時長為1~4年，平均時長為（1.6±0.4）年。將兩組基線資料進行比較，P>0.05。
1.2 方法
1.2.1 對照組：血液透析，血液透析治療選擇4008 s血透機進行，選擇的透析液為碳酸氫鹽，每次透析時長為3 h和4 h，透析血流量控制在220~250 mL/min，選擇肝素進行抗凝，可以結合患者具體情況適當應用鐵劑和促紅細胞生成素進行配合治療；針對患者存在的肺病進行常規抗感染、化痰治療。
1.2.2 研究組：血液透析+百令膠囊。其中血液透析方式同對照組一致，百令膠囊（產自杭州中美華東制藥有限公司，國藥準字為Z10910036）用藥方式如下：每次服用4粒，每天服用3次，堅持8周治療。
1.3 觀察指標[2]：本次研究進行結果分析選擇兩組患者治療效果、尿素氮、血肌酐以及患者各項肺功能指標（FEV1、FEV1/FVC）作為參考指標。其中治療效果評估內容如下：患者臨床癥狀得到改善，且腎功能和肺功能改善程度達50%，為顯效；患者臨床癥狀部分改善，其腎功能和肺功能指標得到明顯改善，為有效；患者臨床癥狀沒有任何改善，其腎功能和肺功能指標沒有任何變化，為無效。
1.4 統計學方法：本研究對本次數據進行SPSS19.0統計學軟件分析，%表示計數數據，進行χ2檢驗；（ ）表示計量數據，進行t檢驗。P<0.05時，組間數據差異具有統計學意義。
